By August 3, authorities documented 3,874 confirmed cases and 1,751 fatalities. This marks Congo’s most extensive recorded Ebola outbreak and ranks as the second largest globally. The country reached 1,000 cases within just 40 days of initiating its response efforts, whereas a significant outbreak starting in 2018 took approximately 235 days to reach that milestone.

Health officials announced the outbreak on May 15 after laboratories identified Bundibugyo virus in Ituri province. The outbreak has extended beyond Mongbwalu into numerous health zones across eastern and northeastern Congo. By July 30, response teams had tracked 17,863 contacts, but follow-up remained inconsistent, particularly in provinces challenged by insecurity and difficult terrain. Moreover, many new patients have been identified outside of known contact networks, indicating that surveillance teams have yet to fully trace all transmission chains.
Ongoing conflict has complicated case detection and patient care, with armed attacks obstructing roads, disrupting health activities, and forcing suspension of field operations. High mobility among populations—who frequently travel between mining sites, markets, towns, and displacement camps—further hampers daily contact monitoring. Healthcare facilities face shortages of protective equipment, trained personnel, transportation, and laboratory access. As of July 30, Congo reported 151 infections and 44 deaths among health workers, increasing the strain on an already overwhelmed response system.
The absence of vaccines and ongoing insecurity accelerate virus spread
Transmission of Ebola occurs mainly through direct contact with the blood or body fluids of infected individuals. This risk is heightened in homes, clinics, and burial settings lacking proper infection control measures. Over 60% of recent fatalities happened outside designated treatment centers, complicating safe burials and contact tracing efforts. In response, Congo’s World Health Organization and Africa CDC have expanded laboratory capacity, established more treatment sites, intensified border screenings, and launched public awareness campaigns. Despite these efforts, containment has struggled to keep pace with the rapid and wide geographic spread of new cases.
While Uganda successfully ended its linked outbreak on July 28 after 42 days without local transmission, and France reported no secondary infections from its single treated case, Congo continues to grapple with persistent transmission. The confirmed death rate remains near 45% as of early August. The faster progression of this outbreak is attributed to late detection and the lack of strain-specific vaccines and treatments. Factors like missed contacts, conflict, healthcare staffing shortages, and population movements have all contributed to increased transmission. These combined challenges distinguish the current Bundibugyo epidemic from previous Ebola outbreaks in Congo.
